The appointment preparation pack is a private workspace for writing down what actually happens in your life, in your words, and turning it into a clear printable set of notes to bring to a GP or specialist.

It is not a screening, an assessment or a diagnosis. It does not score you, rate you or tell you what a clinician will conclude. Every field is optional; anything you leave blank stays blank.

How it works

Six short sittings, not one long form.

  • Confirm you are 18 or older and give an email address for your receipt and a recovery link. That is all we ask for.
  • Pay A$39.00 once through PayPal. Your workspace unlocks as soon as our server confirms the payment with the provider.
  • Fill in whichever sections are useful, in any order. Save when you like; come back any time from the same browser, your account, or an emailed recovery link.
  • Preview and download the PDF of exactly what you saved. Print it or keep it on your phone. If your notes use characters the PDF fonts cannot print (some scripts and emoji), the workspace says so and you print the same saved notes from your browser instead; nothing you wrote is altered.

The free alternative

You do not need this to get help.

The six-question screening, your result and its printable PDF are free and always will be. Your own notebook, or your clinic's intake form, may do the same job. Buy the pack only if a guided, saved and printable version is worth A$39.00 to you.

Refunds and support: ask within 30 days through the help page. After a full refund the editor and PDF switch off, but your notes stay yours to export or delete.

Clear about our limits

What this pack does not do.

  • It does not diagnose ADHD, estimate a likelihood, or produce a score or severity.
  • It does not recommend treatment or medication, and it does not infer anything about your childhood from what you write.
  • It does not promise that a clinician will accept the notes, or that assessment will be faster.
  • It does not use AI to write or interpret your notes. Only what you type appears in your document.
  • It is a writing aid built from plain guidance, not a clinical instrument, and it has not been through clinical review.
